To move forward with learning about your options as it relates to Medicare Advantage Plans (MAPD) or Prescription Drug Plans (PDP), you must complete the highlighted sections on the below Scope of Appointment and return back via USPS, E-mail, or by fax to 773-880-8494.  

NEW MEDICARE BENEFICIARIES

The Health Insurance Shoppe is here to assist newly eligible Medicare Beneficiaries with their enrollment into a Medicare Supplement, Medicare Advantage Plan, and Medicare Prescription Drug Plan (PDP).   There are no fee's to use our service.

If you will be enrolling in Social Security Benefits you'll automatically be enrolled in Medicare Part A and Medicare Part B and once you receive your Medicare Claim Number you can then move forward with your enrollment.

Medicare beneficiaries who will not be enrolling in Social Security Benefits must enroll in Medicare Part A and Medicare Part B through Social Security and recommend completing online through www.ssa.gov when eligible (3 months prior to turning 65).

MEDICARE PART D CONSULTING FEES

For the 2023 Medicare Annual Enrollment Period (AEP) there will be a $150 consultation fee for Part D Coverage.   The fee is applicable only to those who are currently enrolled in a Medicare Part D plan and is per each Medicare beneficiary.
