This "Fee-for-Service" consulation is only applicable to Medicare Beneficiaries during the Medicare Annual Enrollment Period (AEP) that are searching out a standalone Medicare Prescription Drug Plan (PDP).   This does not apply to Medicare Beneficiaries that are currently within their Initial Enrollment Period (IEP) or to current Medicare Advantage Plan enrollees that are looking to change Medicare Advantage Plans that include prescription drug coverage.
 
Consultation is preferred to be in-person at The Health Insurance Shoppe.

Requirements:
Must provide a list of Medications to include exact drug name (please list generic drug name if taking the generic), dosage, monthly quantity, and preferred pharmacy.
